Just from seeing it several times while reading, I understand that it has something to do with (I think) quoting something someone said verbatim, mistakes and all. I always see it after a word is misspelled. Is this the only time it's used? Is it short for something? How/where did it originate?

Our student shorthand for remembering that term in junior high was Spelled InCorrectly. I've never forgotten it! I always think that everytime I come across it in a quote.

I always thought it was "stated in context"
